Care you can trust, so your loved one can stay home, safe and dignified.
When someone you love needs extra support, the decision can feel heavy. We’re here to help you care for them with patience, respect, and cultural understanding. Right in the place they feel most comfortable.
Our approach
You don’t have to do this alone. Caring for a parent, grandparent, or loved one is deeply personal. Whether you’re balancing work, family, or caregiving from a distance, we step in to support both you and the person you care about.
Why families choose us.
Because care should feel human, not clinical. We understand that letting someone else care for your loved one is not an easy decision. That’s why we focus on building trust first, through consistency, communication, and cultural respect.

Understanding CFSS
Community First Services and Supports (CFSS) is a Minnesota program that helps people get support to live in their homes and communities.
CFSS replaced the state’s previous PCA and Consumer Support Grant (CSG) programs in October 2024. It gives participants more flexibility in how they use their approved services and who helps them manage those services.
The services a person can receive are based on an assessment of their needs. CFSS does not automatically provide the same services or amount of support to everyone.
CFSS has two models. Both are designed to help participants use their approved services, but they work differently.
Agency Model
With the Agency Model, a home care agency manages the services. The agency handles responsibilities such as hiring and paying workers. The participant can choose a worker and direct how that worker provides their care.
This model may be a good fit for someone who wants an agency to handle more of the administrative responsibilities.
Budget Model
With the Budget Model, the participant has more responsibility for managing an approved service budget. They can use that budget for eligible supports and may have more control over how services are arranged.
This model may be a good fit for someone who wants more control and is comfortable managing additional responsibilities.
